How to use this tool

At each milestone (1, 3 and 6 months), score every criterion 1–5 on evidence you actually saw — not on a general impression. 1 = no evidence yet, 5 = strong and consistent.
Add a short note per checkpoint with one specific example for the low and high scores. This is what makes the review fair and reviewable later.
Watch the band on each checkpoint (On track / Developing / At risk). If a checkpoint comes back "At risk", act then — a documented support plan beats a surprise at month six.
