New Hire Evaluation & Confirmation Scorecard

Track a new hire fairly across their probation with structured check-ins at 1, 3 and 6 months. Score the same criteria each time, capture evidence, and let the 6-month roll-up give you a clear, defensible confirmation recommendation — instead of a gut-feel decision on the last day.

1
1 MONTH

Settling in

Is the ramp-up healthy? Catch problems early while they are cheap to fix.

2
3 MONTHS

Competence emerging

Are they doing the core job with less and less hand-holding?

3
6 MONTHS

Confirmation

Independent, consistent, here to stay? This drives the decision.

How to use this tool
  1. Enter the hire's name and role once. Everything saves automatically on this device as you go.
  2. At each milestone (1, 3 and 6 months), score every criterion 1–5 on evidence you actually saw — not on a general impression. 1 = no evidence yet, 5 = strong and consistent.
  3. Add a short note per checkpoint with one specific example for the low and high scores. This is what makes the review fair and reviewable later.
  4. Watch the band on each checkpoint (On track / Developing / At risk). If a checkpoint comes back "At risk", act then — a documented support plan beats a surprise at month six.
  5. Use the 6-month recommendation as a structured input to the confirmation conversation, not an automatic verdict. The score tells you where to look; the manager still decides.
